Upon request, here is a picture of a Taoist Spiritual Petition (道教疏文), with the Official format of how to include the Official Title of the Leading Taoist Priest (included in the front 奏職籙號及官階), for the making of Reporting to Higher Authori...ty in the Heavenly Realm, in order to let the Higher Authority to know that who is making this Blessing Requesting in the Mortal Realm.
Besides that, the Spiritual Petition must Closed with the Date of Reporting using the Tian Yun (天運), as this was being standardised by the Ming Dynasty Emperor (明朝皇帝) while bestowing Taoist Zheng Yi Sect to look after the whole of Taoism Society in the World back in Ming Era (正一教永掌天下道教事).
